Along these lines, three documents in the portal should be read and signed electronically: the Notice of Privacy Policies , required by the Health Insurance Portability Accountability Act; a Medicare Opt-Out Contract; and our Practice Policies and Fee Schedule. Please read the Practice Policies and Fee Schedule carefully prior to signing it so that you understand how we operate and how you will be billed.

Four forms need to be manually filled-out: Registration, Medical History, Authorization to Release Health Information, and Authorization to Obtain Health Information. Please bring them to your first appointment. (If you prefer to have these provided to you, please arrive 15 minutes prior to your consultation and we will have them ready for you in the reception area.)
